Required Forms
Following is information about forms that must be submitted to the nurse at the beginning of the academic year, or at the time of entry:

• Kindergarten or original entry into a Pennyslvania public school requires a parent/guardian submit immunization records, a physical, and a dental form. We will accept physical and dental forms dated after September 2011. All paperwork for the 2012-13 school year is due by October 1, 2012.

• A physical is required again in 6th grade. Our school physician will be available in fall 2012 to complete physicals at school. Please sign and return the permission slip to the school nurse by October 1, 2012.

• A dental form is required for 3rd grade and 7th grade students.

• 7th graders will need 2 additional vaccines for the incoming 2012/2013 year. They are:
-1 dose of tetanus, diphtheria, acellular pertussis (Tdap)
-1 dose of meningococcal conjugate vaccine (MCV).

Please submit ALL dental forms, physical forms, and immunization records by October 1, 2012 (this includes ALL grades).

Medications in School
In order for a child to have medications in school, the Authorization for Medications in School form (see above) must be signed by the parent and prescribing doctor, and the medication be given to the nurse in the original container with prescription label on it.

*Any medication that is not brought to school by an adult, is not in the original container, and does not have the required authorization form filled out by the prescribing physician, will not be administered in school.

If you have a child at the school that currently takes daily medications or needs an inhaler or EpiPen at school, please resubmit all forms to the school nurse in the beginning of the 2012/2013 school year. The prescribing physician must re-sign the Authorization for Medications in School form for the new year (2012/2013).

Mobile Dentist
The Mobile Dentist will be available to the students again in 2012-13. Permission slips are available from the Nurse.

Flu Season Guidelines
Please keep your child home if they are experiencing a fever (100°F or higher) accompanied by cough, runny nose, fatigue, sore throat, earache, or rash.  Your child should remain at home until at least 24 hours after they are free of fever, or signs of a fever without the use of fever-reducing medications.

If your child is experiencing more than one episode of diarrhea or vomiting, they should be kept at home. They are allowed to come back to school after 12 hours have passed since the last episode.  

Keeping hands clean through improved hand hygiene is one of the most important steps we can take to avoid getting sick and spreading germs to others (CDC, 2013).
